1) Fill in the form and you will be entitled to take part in our quiz

2) Only one entry per participant  is allowed.

3) Only entries with all correct answers will be qualified.

4) All winners will be notified by email only. 

5) Winners are to bring along their identification cards when they collect the prizes at North West CDC. Thus participants have to give information per their identification cards when registering.

6) Prizes not claimed within one month of the notification date will be forfeited.

7) Judges' decision is final and no correspondence will be entertained. North West CDC reserves the right to change the terms and conditions at its discretion.
